Please bring your valid Health Card with you every time you visit. Please make sure your Health Card is not expired before you come in for your appointment.

Please bring a list of any and all prescription medications as well as vitamins or herbal remedies you’re currently taking.

Please bring a copy of your medical records from your previous doctor, primary care physician or specialist.

If your family physician is not available and you have concerns that cannot wait until he or she is available, you may schedule an appointment with another family physician by walk-in basis.

Yes, you will be asked to present your Health Card on every visit.. A valid Ontario health card must be presented each and every time you visit 101 Medical Centre. If you do you not have a valid OHIP card you will be subject to a fee.

Contact your family physician for a referral. Referrals can be faxed to 905-597-4458.

Yes. Failure to provide a cancellation notice 24 hours prior to a family physician appointment or 48 hours prior to a specialist appointment will result in a fee charge. The fee will varies from $80 to $300 accordingly.

No. Our reception staff is neither allowed nor trained to provide medical advice or interpret the results. If you want to discuss your results please book an appointment with our doctors at 905-597-4457 or request at appointment online.

Please book an appointment with your health care provider to obtain a refill. We believe in quality health care and recommend a consultation prior to renewal to ensure it is safe and effective. In certain situations, which may prevent you from visiting our office request for refill will be reviewed by physicians (the fee of $15 for renewal of your medications with the physician approval may apply).

No. Our reception staff is not allowed to provide medical results over the phone or to provide medical advice. If you want to discuss your results please book an appointment with our doctors at 905-597-4457 or request an appointment online.

We believe in quality health care and recommend a consultation prior to the renewal of medication to ensure all is well. In certain situations, which may prevent you from visiting our office, requests to refill will be reviewed by physicians (the fee of $15 for renewal of your medications with the physician approval may apply).

The form can be filled by our doctors. Please call ahead of your appointment and ensure you let the receptionists know about any forms you may need filled out.

  • If you do not have an OHIP health card you will have to pay for the visit; there are no exceptions.
  • If your OHIP health card expired you have to leave the deposit that will be returned to you after providing renewed health card information ( within a 1 month period.)
  • Specialist appointments have to be confirmed. You have 48 business hours to cancel or reschedule a specialist appointment
  • You have 24 hours to cancel or reschedule a family physician appointment.
  • In order to see the doctor, the patient must be present at the time of registration and appointment.
  • We have zero tolerance for foul language, rude or/and abusive behaviour. If you use foul language or behave in an inappropriate manner you will be asked to leave the clinic immediately. Please be respectful to our staff and doctors.
